Camp Forms
Great Parks uses CampDoc to enhance the day camp experience for participants and staff through its specialized electronic health record system. Parents or guardians must complete their camper's health profile online through CampDoc at least seven days before camp begins. Additional information is included in the confirmation email sent upon registration.
For questions or assistance with CampDoc, visit the CampDoc Support site, email the CampDoc Support Team or call 734-636-1000.
